Hi, I’m Tristan Mohamed, Registered Psychotherapist and owner of Life Transitions Therapy.
My own experience as a trauma survivor and descendant of immigrants navigating the complexities of living ‘between worlds’ led me to seek out deeper healing, meaning, and purpose in life.
I left a career as a not-for-profit lawyer to pursue my passion as a mental healthcare provider as I’ve been in the client’s seat and I know the pain that comes with feeling stuck for way too long.
I started Life Transitions Therapy because I believe everyone deserves to live with confidence, purpose, and joy.

These responses are protective responses when our nervous system is put into overdrive. Therapy helps reduce overwhelm, build coping tools, and restore a sense of balance.

Past experiences can quietly shape how you respond to stress, closeness, and conflict – even when you don’t think of yourself as “traumatized.” Trauma-informed therapy offers a paced approach to understanding these responses without risking re-traumatization. Therapy can help you develop a greater sense of safety, choice, and flexibility in how you experience emotions and relationships.
Loss is not limited to death. Grief can follow relationship changes, health issues, or life shifts. Endings, career changes, identity shifts, or relationship loss can create feelings of isolation that are difficult to articulate. Session fees range from $150-$225 for individual therapy and $325-$350 for couples therapy. Fees are discussed during your initial consultation call.
Yes secure online therapy is available across Ontario. At Life Transitions Therapy, we do our best to ensure sessions are secure. Virtual therapy is held over video calls using JaneApp, a health and privacy legislation compliant platform.
Many extended health plans cover psychotherapy. Please check with your provider for policy and coverage limits.
We believe in more than just a band-aid approach to therapy. Therapy approaches and plans are tailored to your unique needs. We use a combination of modalities including psychodynamic, relational, cognitive behavioural therapy (CT), Gestalt therapies. We also use cutting edge trauma-informed therapies including Internal Family Systems (IFS), somatic therapy, and spiritually integrated therapy.

Most sessions are 50 minutes. Session length and frequency can be discussed during your initial consultation.
Some clients prefer weekly sessions, while others choose bi-weekly or as-needed appointments. Frequency is tailored to your goals and needs, and assessed at your first appointment.
Yes. Many people start therapy feeling overwhelmed or unclear about their goals. Therapy helps explore emotions, patterns, and experiences to gain clarity and direction on your wants, desires, and life goals.
Yes. All sessions are confidential and follow professional and legal confidentiality standards, with limited exceptions required by law.
While each client is different, our holistic and integrative approach to therapy means that results are typically seen within 4-6 sessions.
A standard short course of treatment is 8-12 sessions. Where more complex issues require ongoing treatment, this will be discussed with your practitioner in session.
Starting from downtown Toronto:
Make sure to use a navigation app like Google Maps for real-time traffic updates and the most accurate directions.
There are metered “Green P” street parking spots located along Danforth Avenue – although availability may be limited due to volume – and there is one “Green P” parking lot located nearby.
The closest lot to the office is located at 72 Amroth Avenue. If this lot is full, you can visit the official “Green P” website to find other options.
From Downtown Toronto:
Take the subway to Bloor Station (Line 1 – Yonge-University).
Take the Bloor-Danforth subway line east to Woodbine Station and from there you walk to the office within a few minutes.
Exiting Woodbine subway station, turn left and walk up to Danforth Avenue.
Then turn right and head west past Woodbine Avenue, cross to the south side of Danforth Avenue and then keep heading west just past Moberly Avenue and you will see the “The Healing Collective” sign a few doors down.
Remember to check the TTC website’s schedules and route planning features to tailor your starting location and current time.
Local landmarks to watch out for include Cafe Cocoro, Danforth Carpet Connection and Manzer Hair Studio.
